Test Procedure

How should I position myself before starting the test?
Make sure you follow these guidelines when performing measurements. Failing to do so may lead to inaccurate results or cause the measurements to fail.
Please sit down and relax for 5 minutes before starting the measurements.1. Sit upright with your back straight, your shoulders adducted, and your feet flat on the floor.
2. Bend your dominant arm at the elbow so the upper arm makes a straight angle (90 degrees) with the lower arm. Your forearm should be horizontal to the ground and unsupported as shown in the picture.

3. Close the fingers of your dominant hand around the bulb of the Eforto® Device so the thumb is on one side and the other fingers are on the opposite side.
4. Hold the Eforto® Device so that the wide end of the bulb is at the bottom and the enclosure is at the top.
5. Take the measurement in a calm and quiet area.
6. Stay still, and do not talk. Make sure to keep normal breathing while performing the measurements.
In some situations, this position will not be possible. In this case, a physician should advise you on the position you should adopt for your usage of the Eforto® Device.
Note: Grip strength measurements can be affected by the position of the Eforto® Device and your physiological and emotional condition. -
Can I report the person’s weight in pounds instead of kilograms?
To update the weight measurement unit, please contact us at connect@eforto.com and include
Note: The selected unit will apply to all users and subjects at your site. If your site is part of a multi-site project, the same unit will be used across all sites in that project.

Why can’t I complete the muscle fatigability test?
To begin the muscle fatigability test, you must reach at least 80% of your maximum grip strength. If this level isn’t achieved, the test will not start and is marked as failed.
The test continues until your grip strength falls below 50% of your maximum strength, which you need to maintain for at least 10 seconds to complete the test successfully.

-
Which questions appear before each vitality capacity test?
The questions at the beginning of the vitality capacity test are originating from the Multidimensional Fatigue Inventory (MFI-20). Only the questions for the physical fatigue domain are requested to be filled in.
The answers to these questions are used to calculate the person’s self-perceived fatigue (physical fatigue) and vitality capacity (Capacity‑to‑Perceived Vitality ratio = CPV-ratio).
You can find more information about the MFI-20 here:
Smets, E.M.A., et al., 1995. The Multidimensional Fatigue Inventory (MFI) psychometric qualities of an instrument to assess fatigue. Journal of psychosomatic research, 39(3), pp.315-325. DOI: 10.1016/0022-3999(94)00125-o -
Can I update the vitality capacity test protocol to include the full MFI-20 questionnaire?
Yes, the vitality capacity test is only one of the protocols that Eforto® offers. The “multi-fatigue test” protocol requires you to always fill in all 20 questions of the Multidimensional Fatigue Inventory (MFI-20) and calculate the corresponding scores.
You can request an update of your test protocol by sending an email to connect@eforto.com.The request to update the protocol should include the following information:
-
The site to which you belong (i.e. the company/hospital/...etc. that bought the Eforto® Device)
-
The protocol(s) that you want to use.
Note 1: The available protocol(s) is/are the same for all users and all subjects of the site.
Note 2: It is possible to have access to multiple protocols at the same time.
Note 3: If you are part of a project (with multiple sites), all sites of the project share the same protocol(s).
-
-
Can I remove the four questions before the vitality capacity test?
Yes, the vitality capacity test is only one of the protocols that Eforto® offers. The “Muscle capacity test” protocol does not contain any questions from the Multidimensional Fatigue Inventory (MFI-20). It only assesses grip strength and muscle fatigability.
You can request an update of your test protocol by sending an e-mail to connect@eforto.com. The request to update the protocol should include the following information:
-
The site to which you belong (i.e., the company/hospital/... that bought the Eforto® Device)
-
The protocol(s) that you want to use
Note 1: The available protocol(s) is/are the same for all users and all subjects of the site.
Note 2: It is possible to have access to multiple protocols at the same time.
Note 3: If you are part of a project (with multiple sites), all sites of the project share the same protocol(s).

Results & Interpretation
-
Why do I have a low grip work result?
Grip work is calculated as the area under the time-pressure curve during the muscle fatigability test. It reflects the total physiologic work produced by your handgrip muscles while maintaining force over time.
Two key factors influence your grip work value:
-
Duration of the muscle fatigability test: how long you can sustain at least 50% of your maximum strength.
-
Overall grip strength: your force level during the test.
A low grip work result usually means one of the following:
-
You could only hold 50% of your maximum grip strength for a short period.
-
Your overall grip strength is low.
You can learn more about test interpretation and ways to improve grip performance here: Muscle Fatigability
-
-
What criteria are used to compare my values to the reference values?
Results cutoff points are calculated based on biological sex and weight compared with standardized normative data.
These reference data are driven from research conducted with academic partners and validated cohorts with adjusted thresholds for muscle strength, muscle fatigability, and capacity-to-perceived vitality ratio (CPV ratio). You can learn more about test interpretation and ways to improve grip performance here:
Grip Strength -
Why do my muscle fatigability results vary during the day?
Muscle fatigability can fluctuate throughout the day due to natural changes in muscle fatigue, hydration, motivation, or temperature.
Measurements are also affected by the way you hold the Eforto® Device and your measurement position. For consistent results, measure at the same time each day and under similar conditions. If you are under treatment or prescription of drugs, your handgrip strength and muscle fatigability readings may vary more. -
Is the result the same if I use my left or right hand?
The result may differ depending on which hand you use. It is recommended to perform the test with your dominant hand, as the color-coded feedback is based on normative data for the dominant hand. People generally do not have the same strength in their dominant and non-dominant hands, so using your non-dominant hand may lead to lower or inconsistent results.
-
Does hand size affect the test results?
No, hand size does not affect Eforto® test results. The Eforto® system is validated using bulb size 5, and all normative data are based on this standard size.
Research has shown that the Vigorimeter (used by Eforto®) is unaffected by hand geometry, meaning that measurements remain accurate regardless of hand size or shape. This makes the Eforto® Device a more reliable tool for assessing grip strength across diverse populations compared to traditional dynamometers, which can be influenced by anatomical differences.

Can I see my individual grip strengths?
Yes, you can view the grip strength of each attempt in the detailed results.
Simply tap the icon in the upper-right corner of the Results screen to open the detailed view of your test.
In the “Grip strength” section, you’ll find the values for each grip strength test.
Please note that grip strength 4 represents the maximum grip strength reached during the first five seconds of the muscle fatigability test.

-
Can I convert the grip strength in kPa to kg?
Eforto® measures dynamic grip strength in units of pressure, specifically kilopascals (kPa), which represent force per unit area. While we do have a conversion factor to kilograms, the measurement dynamics of the Eforto® Device differ from those of hydraulic systems. The Eforto® App and Eforto® Metrics can automatically display estimated values in kilograms, which is useful at the population and cohort levels but less advisable for individual subjects, as the conversion is based on an estimation.
One of the causes of estimation error can be due to the fact that measurements with the traditional dynamometers are more influenced by the morphology of the hand compared to the bulb-based pneumatic Eforto® Device/Vigorimeter.
We have validated normative data for the Eforto® Device for grip strength with thresholds/cutoffs for sarcopenia, as well as for muscle fatigability and CPV ratio as an indicator of Vitality Capacity and early signs of Frailty.
The conversion formula to go from a result in kPa to a result in kg is as follows:
Estimation in kg = result in kPa * 0.43
-
Where can I see my grip strength in kg?
By tapping on the Results screen on the icon in the upper right corner, you can view the details of your test. In the section “Grip strength”, you can view each individual grip strength and its conversion to kg.
